the membership card of Association cannot be issued unless he or she co-payment the fees of a membership
after the Commission of Association approved for Membership, and the Membership has pay the fees required it will be issued membership card within 30 days from the date of approval.
The registration with incorrect information in this form will cause the rejection of membership of this Association.
